膵臓腺がんはほぼ100%の致死率で早期発見が不可能で一部の患者の遺伝要因以外には原因は不明である。関連が示唆されているものは喫煙や大量飲酒、高脂肪食、炭酸飲料の飲み過ぎ、肥満、慢性膵炎、糖尿病などがあるが疫学調査では矛盾した結果が出ている。IARCの2009年モノグラフでは膵臓がんと飲酒の関連は根拠が不十分とされた。
この解析では喫煙と大量飲酒（1日に39g以上あるいは3-4杯）が早期発症と関連することを示した。
